In this video, the Mayo Clinic provides an easy to understand animated description of the mechanics of the Roux-en-Y gastric bypass procedure.  You'll see how a small stomach pouch is made and attached to the small intestine, and how the intestine attached to the main stomach is reattached to the small intestine further down, creating the "Y" shape.

Roux-en-Y gastric bypass may be done in one of two ways: open surgery and laparoscopic surgery.  Open surgery requires a large incision in the abdomen to allow the surgeon access to the stomach and intestines.  Laparoscopic surgery uses small access points through which a small camera and surgical instruments enter the patient’s abdomen.  The surgeon then performs the procedure by viewing what he does via a monitor.  Laparoscopic surgery generally allows for a quicker recovery for the patient.
